Admission for Rehabilitation Care

When admitted from the hospital after a three-night qualifying stay, Medicare A benefits and your supplemental insurance will pay for room, board, therapy, medications, lab work, and x-rays, per Medicare guidelines.

The hospital Discharge Planner may provide a list of facilities to fit the individual’s rehabilitation needs.  Families are encouraged to tour the facilities and inform the Discharge Planner of their decision. When you have selected ACC Care Center, you can request that the referral be sent to your community of choice, and the hospital will take it from there and coordinate the transfer.

Admission for Continuing Care

Continuing Care or long-term care is based on availability. Those interested in placement should contact the Admissions Coordinator at (916) 393-9020 ext. 249 for more information.
